Patient Centered Care Coordinator
She overseas managing the health care needs of clients, which generally involves coordinating with health services, communicating with members of the care team, and directing client to services and assistance program available to them.
The Duties and Responsibilities of PCCC:
- Assist patient from admission to discharge to ensure that patients received the needed care.
- Orient patient and significant others regarding our facilities, room rates, services offered and financial assistance program available to them.
- Assists patient to settle their hospital bill.
- Conduct interview to patient prior to discharge for continuous quality improvement.
- Inform patient that his/her home medications are ready for pick up at the New pharmacy.
- Gather and file all the documents such as survey forms, reports and demographic information of the patient in order to analyze their feedback regarding our services.
- Schedule patient appointment and follow -up check up.

V.L. Makabali Memorial Hospital Inc. has set up a health desk in the OPD area for patients to receive free blood pressure and free fasting blood sugar FBS. Every day, 20 to 30 people visit to the health desk to take advantage of the hospital’s free medical services.
Information Desk
The information desk service welcomes families, patients, visitors, and staff with respect and compassion. The duties of Information desk are to Maintaining updated patient lists in the hospital. Provide information about patients’ room numbers and visiting hours, Direct guests to appropriate hospital departments or offices. and to provide the schedule Doctor clinics for the patients.
